I give permission and consent to the Cancer Association of South Africa (CANSA) to release my story (including information about my diagnosis) and photo. I understand that the story and photo may be used in newspapers, magazines, CANSA website, CANSA online and social platforms, online media, television and film for the purpose of creating public awareness about lowering the risk of cancer and promoting CANSA and its service and programmes. I am aware that, I will not receive payment.

Encourage other cancer patients by sharing your experience and advice - what helped you cope with your diagnosis, cancer treatment, and if applicable, being in remission?

Guidelines for photos:

Ensure you use landscape / horizontally format
Have the light in front of you, no backlight – avoid placing bright lights, windows or other light sources directly behind you
Do a head and shoulders shot, don’t film the whole body (try not to be in the middle of the frame).
Try and stand in a way that is relaxed, with your body angled slightly away from the camera. Fold your arms, have your hands on your hips, or put your hands in your pockets — whatever feels most natural. Remember to keep your shoulders back and your chin up.
Look directly in the camera and please smile
Try and pick a plain background, a wall or other no, bookshelves etc. we aiming to create a library of pics that make the survivor the main feature
Please ensure lens is clean
